1.	Birth Certificate
2.	ID Card/Residence Permit
3	Proof of Address
4.	SC or equivalent
5.	HSC or equivalent
6.	Passport
7.	Evidence or Pre-registration Training
8.	Degree/Diploma in Medicine (to submit official translation in English or such wherever applicable, and mark sheets or transcripts, etc…)
9.	Valid Work Permit (for applications for temporary registration)
10.	Latest Certificate of Good Standing from Foreign Medical Council/Board
11.	Latest Registration Certificate (for those persons who have practiced abroad)
12.	Evidence of work experience for at least 3 years after being registered at a Council
